Make a claim
You can make a claim online, via email or by post.
TO Make a claim online
1. Log in to the PG Mutual Member Area
If you have already registered with Member Area, you can simply log in here. If you need to register, it’s a quick process. You will just need your customer number, surname and date of birth.
2. Once in the members area, click on ‘Your Claims’ and then ‘Make a Claim’ and follow the on-screen instructions to submit your claim.
It is important that you notify us of a claim as soon as you can regardless of your deferral period.
Please make sure that you also provide us with appropriate supporting documentation of your incapacity and your earnings. For example, a Fit Note from your doctor and a recent pay slip or bank statement showing your income. These can be scanned or photographed and provided to us via email to info@pgmutual.co.uk, or attached to your online claim at the end of form.
Please be aware that if you don’t provide us with your completed claim form and supporting evidence within 90 days of the date you were eligible to receive benefit, you may forfeit income benefit you would otherwise have been entitled to.
3. Assessment of your claim
Once received, we will assess your claim individually. In some instances, we may request further information which we will notify you of.
4. Approved claims benefit paid
Your benefit will be paid directly into your personal bank account once approved. Please note that we are unable to make claim payments into business bank accounts.

1. Get a claim form
For a physical claim form, call us on 0800 146 307 or you can download one here.
2. Fill in your claim form and send it to us with your supporting documents
It is important that you notify us of a claim as soon as you can.
